En randomiseret klinisk undersøgelse belyser økologiske effekter af en tandpasta med enzymer og proteiner på plakkens mikrobiom

Vi ved nu, at de mange arter, der udgør det orale mikrobiom, spiller en nøglerolle for opnå­else og vedligeholdelse af oral sundhed. Mulig­heden for at kunne foretage taksonomisk be­stemmelse af mikrobiomet på artsniveau er en forudsætning for at kunne beskrive dets diver­sitet og dets sammenhæng med sundhed og sygdom. Vi rapporterer her, hvilke økologiske virkninger brugen af tandpasta med enzymer og proteiner har på plakkens mikrobiom sam­menlignet med en kontroltandpasta. Resulta­terne påviser, at en tandpasta, der indeholder enzymer og proteiner, kan fremme spyttets naturlige forsvarsmekanismer og fremkalde en ændring i mikrobiomets sammensætning med et øget antal af bakterier, der er forbun­det med parodontal sundhed, og et samtidigt fald i antallet af bakterier, der er forbundet med parodontal sygdom. Den statistiske analyse viser signifikant forøgelse af 12 arter, der er forbundet med parodontal sundhed, heriblandt Neisseria­arter, og signifikant reduktion af 10 arter, der er forbundet med parodontal syg­dom, heriblandt Treponema arter. Resultaterne viser, at en tandpasta, der indeholder enzymer og proteiner, signifikant kan ændre det orale mikrobioms økologi (på artsniveau), så mikro­biomet fremtræder mere sundt.

A randomised clinical study to determine the effect of a toothpaste containing enzymes and proteins on plaque oral microbiome ecology
The numerous species that make up the oral microbiome are now understood to play a key role in establishment and main-tenance of oral health. The ability to taxonomically identify com-munity members at the species level is important to elucidating its diversity and association to health and disease. We report the overall ecological effects of using a toothpaste containing en-zymes and proteins compared to a control toothpaste on the plaque microbiome. The results reported here demonstrate that a toothpaste containing enzymes and proteins can augment natural salivary defences to promote an overall community shift resulting in an increase in bacteria associated with gum health and a concomitant decrease in those associated with periodontal disease. Statistical analysis shows significant increases in 12 taxa associated with gum health including Neisseria spp. and a sig-nificant decrease in 10 taxa associated with periodontal disease including Treponema spp. The results demonstrate that a tooth-paste containing enzymes and proteins can significantly shift the ecology of the oral microbiome (at species level) resulting in a community with a stronger association to health.

KLINISK RELEVANS
Undersøgelsens data be­kræfter, at en tandpasta, der er sammensat med henblik på at styrke spyttets naturlige forsvarsmekanismer, medfø­rer positive ændringer i mikro­biomet, som er mere forene­lige med sundhed. Nærmere betegnet påvises det, at en tandpasta med enzymer og proteiner over tid kan føre til en forøget relativ forekomst af sundhedsrelaterede bakterier i plak og samtidig en faldende forekomst af sygdomsrelate­rede bakterier.
