Karcinom i mundslimhinden – tidligere diagnostik er mulig

Baggrund – Planocellulært karcinom er langt den hyppigste cancerform i mundslimhinden. Typiske kliniske tegn er et ikke-helende sår, ofte med voldformede rande samt induration ved palpation, men også røde, hvide og blandet rødhvide, evt. nodulære forandringer bør vække mistanke – også i tilfælde uden ulceration. Patienttilfælde – Hos en 32-årig mand konstaterede egen tandlæge en kraterformet ulceration med voldformede rande i højre kindslimhinde. På mistanke om bidlæsion afventede egen tandlæge effekten af ekstraktion af en tand (8+) som mulig årsag til forandringen. Imidlertid var tilstanden ikke bedret syv måneder efter ekstraktionen, hvorefter patienten henvistes til Tandlægeskolen i København for udredning og behandling. Biopsi viste planocellulært karcinom. Konklusion – Patienter med typiske tegn på malign tumor i mundslimhinden bør omgående henvises for udredning og behandling efter gældende retningslinjer. Såfremt en behandling udføres på grund af forventet årsagssammenhæng, skal effekten heraf kontrolleres efter 7-10 dage og henvisning ske, hvis der ikke har været tilfredsstillende effekt.

Oral carcinoma – early diagnosis is possible: Background – Squamous cell carcinoma is by far the most common type of cancer in the oral mucosa. Clinically typical signs are a persistent ulceration, often with raised margins and induration on palpation, but red, white and mixed red and white, nodular apperances should also raise suspicion – with and without ulceration. Case study – A 32 year old man was referred 7 months after an attempt to heal a suspected traumatic lesion in the right buccal mucosa. A biopsy at the Department of Odontology in Copenhagen revealed a squamous cell carcinoma. Conclusion – Patients with obvious signs of a malignant tumor in the oral mucosa should be referred without delay. If treatment of a possible traumatic cause is instituted, the effect should be monitored over 7-10 days and referral immediately carried out if the attempted treatment has no effect.
