Osteoradionekrose (ORN) i kæberne er en af de mest belastende og frygtede komplikationer til højdosisstrålebehandling i hoved- og halsområdet. Derfor forsøges ved fokussanering før strålebehandling at forebygge enhver form for oral kirurgi efter strålebehandling - herunder tandudtrækninger, som ofte er den udløsende årsag til ORN. Artiklen opsummerer strålebehandlingens biologiske skadevirkninger på kæberne, idet dette skal give den forståelsesmæssige baggrund for præirradiativ fokussanering.

Osteoradionecrosis is one of the most serious complications after high dose radiotherapy for cancer in the head and neck region. The risk of overt osteoradionecrosis may be reduced by timely eradication of potential and actual infectious foci before radiotherapy. The protocol for therapeutic management of head end neck cancer in DK is brief- ly reviewed. The paper reviews the literature on pre-irradiation elimination of dental infectious foci. No solid evidence based literature on the subject has been found. Guidelines are given for pre-irradiation dental treatment based on best practice.
