Medicinske sygdomme med relevans for tandlægenskliniskehverdag
Hypofysen regulerer funktionen af binyrerne, glandula thyroidea og gonader og frisætter derudover væksthormon og prolaktin. Mange af de secernerede hormoner er livsvigtige, og i oversigten gennemgås de hormonsygdomme, som specielt kan have relevans for tandlæger. I nogle tilfælde udgør symptomer fra kæbe og mundhule kardinalsymptomer for netop disse sygdomme. Akromegali betegner øget produktion af væksthormon og er forbundet med vækst af kæber og tunge samt tandmigration. Disse symptomer kan betyde, at tandlægen er patientens primære kontakt til sundhedsvæsnet. Binyrebarkhormonmangel, overproduktion af binyrebarkhormon (Cushings syndrom) og behandling med binyrebarkhormon udgør væsentlige problemstillinger for tandlægen. Ved Cushings syndrom ses øget infektions- og blødningstendens fra mundhulen samt ofte oral candidiasis. Det er væsentligt for tandlægen at kende til forholdsreglerne for behandling af patienter med binyrebarkinsufficiens. I artiklen gives anbefalinger om doseringsregimer for hydrokortison ved mindre og større kirurgiske indgreb.
Medical diseases of relevance for the dentist’s clinical work The pituitary gland regulates adrenal, thyroid and gonadal function and secretes growth hormone and prolactin. Several of the secreted hormones are vital and in the present overview we summarize the hormonal conditions of special relevance for the dentist. In some cases, symptoms from jaw and mouth are cardinal symptoms for these illnesses. Acromegaly is defined as increased growth hormone secretion and is associated with growth of the jaw and tongue and migra tion of teeth. Due to the characteristic symptoms of acromegaly the dentist may have an important role in diagnosing this condition. Adrenal hormone insufficiency, adrenal hormone overproduction (Cushing’s syndrome) and treatment with adrenal hormones are important issues for the dentist. In Cushing’s syndrome the patient has increased risk of oral infections, bleeding and oral candida infections. It is important for the dentist to be familiar with the guidelines for medical treatment of patients with adrenal insufficiency. In this article we give recommendations on dosage regimens for cortisol treatment during minor and major surgical procedures.
